Our Health & Wellbeing service is focused on shifting from reactive to proactive care, empowering communities to lead their own approaches to health and using technology to make support more accessible and personalised.
We work to promote holistic wellbeing, deliver targeted support where it’s needed most, strengthen integration across services and ensure long‑term sustainability through evidence‑based decision making and meaningful data insight
We offer three key areas of support:
Community Health
Practical support to help people live healthier lives, including:
- exercise referral programmes
- specialist exercise classes
- health checks
- support for healthy eating
Family Wellbeing
Support for families and children across a wide range of needs, including:
- Play Therapy in schools
- Pre‑5 play sessions
- support for Community Justice
- work within the prison
- support for looked‑after and accommodated young people
Community Wellbeing
Our focus is to support people in our communities to connect, laugh and thrive, living their best life. We deliver activities for older adults, adults with Learning disabilities and those with poor mental wellbeing, to enable them to live a meaningful and resilient life. We do this through a team of dedicated staff who link people to local supports and community groups, volunteer befriending and walk leaders and a wide variety of opportunities for building meaningful connections.
